Employee Benefits

Ontario Systems' employees receive a competitive compensation package that consists of cash (base pay and incentive pay), stock equity and benefits.

The company's benefits program addresses both the immediate needs a family may have, including medical coverage or dependent care referral services, and their long-term needs, such as retirement savings.

The company provides flexibility in its program through such benefits as company bonuses, flexible spending accounts and short-term disability pay.

The following benefits summary applies to regular full-time employees. Part-time employees may have different benefits and costs. Consult with a hiring representative for more information.

Casual Dress Code

Ontario Systems maintains a relaxed, business casual dress code, which includes jeans, shorts, cargo and crop pants.

Medical Insurance

Ontario Systems offers a comprehensive Preferred Provider Organization (PPO) medical plan to employees. With this plan, benefits are paid at a higher level when employees use healthcare providers and medical services within the PPO network. Employees pay a $25 co-payment for office visits to their primary care physician. For visits to specialists and other major medical services, the plan pays 80 percent of covered expenses after a $500 deductible ($1,000 maximum per family) is met. Employees pay approximately 15 percent of the cost of the monthly insurance premiums through payroll deduction. Eligibility begins on the first day of employment.

Prescription Card

Prescription drug coverage is included in the medical plan on a co-payment basis. The employee portion of the cost of covered prescriptions is $15 for generic medications and $30 for brand name medications (up to a 34-day supply). For recurring prescriptions, a 90-day supply of the prescribed medication can be filled through a mail order program. When using the mail order service, the cost for generic medications is $30 and $60 for brand name medications.

Dental Insurance

The dental plan also is through a PPO network. The plan pays 100 percent of preventative services. After a $50 deductible ($150 maximum per family) is satisfied, the plan covers basic services at 80 percent and major services at 60 percent. The cost of this benefit is included in the monthly insurance premium as referenced in the Medical Insurance section. Eligibility begins on the first day of employment.

Vision Insurance

As with the medical and dental programs, employees realize a greater benefit from this plan when they use service providers within the PPO network. Vision exams are covered in full every 12 months when using providers within the PPO network. Lenses or contact lenses are covered every 12 months. Eyeglass frames are covered every 24 months. The cost of this benefit is included in the monthly insurance premium as referenced in the Medical Insurance section. Eligibility begins on the first day of employment.

Flexible Spending Accounts

Health Care Spending Accounts and Dependent Care Spending Accounts allow employees to pay for eligible expenses with pre-tax dollars. Employees may enroll in these accounts each year during benefits open enrollment, typically in November or December. Both accounts are regulated by the IRS.

Life Insurance

Ontario Systems provides group life insurance coverage at an amount of two times an employee’s annual salary. This benefit is currently provided at no charge to the employee and becomes effective on the employee’s hire date.

Short-Term Disability

Full-time employees who are employed for one month or more may be eligible for salary continuation benefits during a leave of absence under Ontario Systems’ Short-term Disability Plan. The Short-term Disability Plan is offered to continue at least a portion of compensation during periods in which the employee is unable to work due to a medical disability. This benefit is currently provided at no charge to the employee.

Long-Term Disability

This plan provides salary continuation benefits for employees who have been disabled longer than six months. Employees may receive up to 60 percent of their salary during the disability period until age 65. This benefit is currently provided at no charge to the employee. Eligibility begins on the first day of employment.

401(k) Savings Plan

This retirement savings plan allows participants to save a portion of their earnings on a tax-deferred basis. Employees may contribute up to 75 percent of their pre-tax earnings into a pre-tax 401(k) account or a Roth 401(k) account. The contributions can be allocated among 19 mutual funds. After an employee completes one year of service, Ontario Systems makes a “basic” contribution of two percent of the employee’s annual earnings to his/her account. Ontario Systems also makes a “matching” contribution of up to two percent of the employee’s earnings per year. Full vesting occurs at three years of employment.

Employee Equity Appreciation Program

This program allows employees who meet certain participation requirements to share in the growth in value of Ontario Systems – without personal financial contribution. Full-time or modified schedule employees who complete 12 consecutive months of service and who remain gainfully employed are eligible to participate.

Cash Incentive Compensation Plan

Once the minimum pre-tax profit goal is attained, this plan pays a percentage of all additional profits into an incentive compensation pool that is distributed on a quarterly basis. Individual allocations of the pool are calculated by summing the salaries of all eligible employees and giving each participant a pro-rata share. Employees are eligible at the beginning of the quarter after one year of service and must be non-officer/non-commissioned employees.

Tuition/Continuing Education Reimbursement

Ontario Systems provides financial assistance to employees who wish to continue their education either through an accredited college-level program or company approved training institute. Areas of study must be job-related. Interested employees must follow appropriate approval procedures.

Paid Time Off

Paid Time Off (PTO) provides full-time and modified schedule employees with paid time away from work that can be used for vacation, personal time, illness or time off to care for family members. New employees earn PTO days at a rate of 1.5 days for each month of service. PTO days not used by the end of the calendar year carry over to the next calendar year up to a maximum of 42 days.

Holidays

Ontario Systems’ employees receive paid time off for specified annual holidays. Additionally, employees receive one floating holiday that can be taken on an approved government or religious holiday of the employee’s choice with advance supervisor approval.

Fitness Center

Ontario Systems offers all employees and their eligible family members access to an on-site fitness facility. The center is available 24 hours a day, seven days a week.

Employee Assistance Program

Employees dealing with the pressures of everyday life may need to seek objective, confidential assistance from a trained counselor. The LifeBalance Employee Assistance Program offers telephone consultations with specialists who can provide assistance with personal problems and relationships; parenting, family and work-balance issues; legal or financial concerns; child care or elder care matters; or substance abuse/addiction issues. Employees may also request referrals to local professionals for one-on-one consultations. The first three visits to professionals dealing with mental health or substance abuse issues are at no charge to the employee. Subsequent visits would be paid according to the benefits outlined in Ontario Systems’ health insurance program.
